Additionally certain pragmatic trials lack controls to ensure that the observed differences are not due to biases in trial conduct.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatic and that were published until 2022. The PRECIS-2 tool was employed to assess the pragmatism of these trials. It includes domains such as eligibility criteria, recruitment flexibility, adherence to intervention, and follow-up. They discovered that 14 of the trials scored as highly or pragmatic pragmatic (i.e. scores of 5 or higher) in any one or more of these domains, and that the majority of these were single-center.

Trials with a high pragmatism rating tend to have broader eligibility criteria than traditional RCTs, which include very specific criteria that aren't likely to be used in the clinical setting, and contain patients from a broad variety of hospitals. The authors claim that these characteristics could make pragmatic trials more effective and useful for daily practice, but they do not guarantee that a trial using a pragmatic approach is completely free of bias. The pragmatism characteristic is not a fixed attribute; a pragmatic test that does not have all the characteristics of an explanatory study can still produce valuable and valid results.
