A lot of pragmatic trials are restricted by the necessity to recruit participants in a timely manner. Certain pragmatic trials lack controls to ensure that any observed differences aren't due to biases during the trial.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatic and were published up to 2022. The PRECIS-2 tool was used to determine the degree of pragmatism. It includes areas like eligibility criteria as well as recruitment flexibility as well as adherence to interventions and follow-up. The pragmatism principle is not a fixed attribute and a test that doesn't have all the characteristics of an explicative study may still yield reliable and beneficial results.
