However, pragmatic tests may be prone to limitations that undermine their effectiveness and generalizability. The participation rates in certain trials could be lower than expected because of the healthy-volunteering effect, financial incentives or competition from other research studies. Many pragmatic trials are also restricted by the need to enroll participants quickly. In addition some pragmatic trials do not have controls to ensure that the observed differences are not due to biases in trial conduct.

The authors of the Pragmatic Free Trial Meta identified RCTs published from 2022 to 2022 that self-described as pragmatic. The PRECIS-2 tool was used to assess pragmatism. It includes domains such as eligibility criteria, recruitment flexibility and adherence to intervention and follow-up. They discovered that 14 of the trials scored highly or pragmatic pragmatic (i.e. scoring 5 or higher) in one or more of these domains and that the majority of them were single-center.

Trials that have a high pragmatism score tend to have higher eligibility criteria than traditional RCTs which have very specific criteria that aren't likely to be present in clinical practice, and they include populations from a wide variety of hospitals. These characteristics, according to the authors, can make pragmatic trials more useful and applicable in everyday clinical. However, they don't guarantee that a trial will be free of bias. The pragmatism principle is not a definite characteristic the test that doesn't have all the characteristics of an explicative study may still yield reliable and beneficial results.
