Because mesothelioma is a chronic disease with an extended latency period, victims can sometimes be diagnosed with peritoneal mesothelioma decades after their initial exposure.

Peritoneal mesothelioma symptoms can appear similar to other diseases and could be mistaken for a hernia, gastrointestinal issues like colitis or Crohn's disease, or other types of abdominal cancer. Many patients do not disclose their asbestos exposure, and do not seek medical attention. Doctors may suggest a biopsy to confirm the diagnosis of mesothelioma. A biopsy is the removal of the smallest piece of tissue to be tested. This test will reveal whether the mesothelioma has epithelioid (epidermoid) or Sarcomatoid (sarcomatoid) or biphasic.

If mesothelioma has not spread it is possible to remove the tumors. They can also provide a curative treatment. The doctor will also be able to determine whether the cancer has spread to the abdomen. They can then prescribe palliative care for the patient, for example paracentesis to drain the fluid from the abdomen.

If mesothelioma progresses and the prognosis is less favorable, it becomes more difficult to treat. The tumors could become resistant to treatment and may recur. The doctor may suggest the combination of chemotherapy and surgery. Surgery options include cytoreduction with hyperthermic intraperitoneal chemo. This combination is more likely to improve a patient's prognosis than chemotherapy alone.
