Pragmatic Free Trail Meta is an open data platform that enables research into pragmatic trials. It collects and distributes cleaned trial data, ratings, and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses that compare treatment effect estimates across trials of different levels of pragmatism.
Background
Pragmatic trials are becoming more widely recognized as providing real-world evidence to support clinical decision-making. However, the use of the term "pragmatic" is not consistent and its definition and assessment requires clarification. The purpose of pragmatic trials is to guide the practice of clinical medicine and policy decisions, not to prove a physiological or clinical hypothesis. A pragmatic trial should aim to be as close as possible to the real-world clinical practice that include recruiting participants, setting up, delivery and execution of interventions, determining and analysis results, as well as primary analyses. This is a major difference between explanatory trials as defined by Schwartz & Lellouch1 which are designed to prove a hypothesis in a more thorough way.
Truely pragmatic trials should not blind participants or clinicians. This can lead to bias in the estimations of treatment effects. The pragmatic trials also include patients from different healthcare settings to ensure that the results can be applied to the real world.
Finally, pragmatic trials should focus on outcomes that are crucial to patients, such as quality of life or functional recovery. This is particularly important when it comes to trials that involve the use of invasive procedures or potentially dangerous adverse events. The CRASH trial29, for instance, focused on functional outcomes to compare a 2-page case-report with an electronic system for monitoring of hospitalized patients with chronic heart failure. Similarly, the catheter trial28 used urinary tract infections caused by catheters as its primary outcome.
In addition to these aspects, pragmatic trials should minimize the procedures for conducting trials and requirements for data collection to reduce costs and time commitments. Finally pragmatic trials should try to make their results as applicable to clinical practice as is possible by making sure that their primary analysis follows the intention-to treat approach (as described in CONSORT extensions for pragmatic trials).
Despite these guidelines, a number of RCTs with features that defy pragmatism have been incorrectly self-labeled pragmatic and published in journals of all types. This can lead to false claims of pragmatism and the term's use should be standardised. The creation of the PRECIS-2 tool, which provides an objective standard for assessing practical features is a good initial step.
Methods
In a pragmatic study it is the intention to inform policy or clinical decisions by demonstrating how an intervention could be integrated into routine care in real-world situations. This is distinct from explanation trials that test hypotheses regarding the causal-effect relationship in idealized conditions. In this way, pragmatic trials may have lower internal validity than studies that explain and be more susceptible to biases in their design as well as analysis and conduct. Despite these limitations, pragmatic trials can be a valuable source of information for decisions in the context of healthcare.
The PRECIS-2 tool assesses the level of pragmatism that is present in an RCT by assessing it on 9 domains ranging from 1 (very explicative) to 5 (very pragmatic). Additionally, 36% of the 89 pragmatic trials discovered by Koppenaal et al were placebo-controlled or conducted before licensing and most were single-center. Therefore, they aren't very close to usual practice and are only pragmatic when their sponsors are accepting of the lack of blinding in these trials.
Additionally, a typical feature of pragmatic trials is that the researchers try to make their results more valuable by studying subgroups of the trial sample. The overall score was lower for pragmatic systematic reviews when the domains on the organization, flexibility of delivery and follow-up were merged.
It is crucial to keep in mind that a pragmatic study does not necessarily mean a low-quality study. In fact, there is an increasing number of clinical trials that employ the term "pragmatic" either in their abstract or title (as defined by MEDLINE but which is not precise nor sensitive). Conclusions
In recent years, pragmatic trials are becoming more popular in research as the value of real-world evidence is becoming increasingly acknowledged. This approach can help overcome limitations of observational studies, such as the biases that arise from relying on volunteers and limited availability and the variability of coding in national registry systems.
Pragmatic trials have other advantages, including the ability to draw on existing data sources and a greater chance of detecting significant distinctions from traditional trials. However, they may still have limitations that undermine their validity and generalizability. Participation rates in some trials may be lower than expected due to the health-promoting effect, financial incentives or competition from other research studies. Many pragmatic trials are also restricted by the need to enroll participants on time. Additionally some pragmatic trials don't have controls to ensure that the observed differences are not due to biases in trial conduct.
The authors of the Pragmatic Free Trial Meta identified RCTs that were published between 2022 and 2022 that self-described as pragmatic. The PRECIS-2 tool was used to assess the pragmatism of these trials. It includes areas such as eligibility criteria as well as recruitment flexibility and adherence to intervention and follow-up. They found that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or above) in at least one of these domains.
