Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It collects and distributes clean trial data, ratings and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses that evaluate the effects of treatment across trials of various levels of pragmatism.
Background
Pragmatic studies are increasingly acknowledged as providing evidence from the real world to support clinical decision-making. However, the use of the term "pragmatic" is inconsistent and its definition and evaluation requires clarification. Pragmatic trials must be designed to guide clinical practice and policy decisions, not to confirm a physiological or clinical hypothesis. A pragmatic study should strive to be as close as possible to the real-world clinical practice which include the recruitment of participants, setting up, implementation and delivery of interventions, determining and analysis results, as well as primary analyses. It is important to understand that the term "pragmatic trial" does not necessarily mean a low-quality trial, and indeed there is an increasing rate of clinical trials (as defined by MEDLINE search, but it is neither specific nor sensitive) which use the word "pragmatic" in their abstracts or titles. The use of these words in abstracts and titles may suggest a greater awareness of the importance of pragmatism, but it is unclear whether this is evident in the content of the articles.
Conclusions
In recent times, pragmatic trials are becoming more popular in research as the value of real world evidence is becoming increasingly acknowledged. They are clinical trials randomized that compare real-world care alternatives rather than experimental treatments under development.
